RESUMO

Results of surgical treatment of 28 adolescent girls with vaginal aplasia and functioning uterus are analyzed. The uterus had to be removed in all the patients 1.5 to 6 months after abdominal colpopoiesis with preservation of functioning uterus; after sigmoidal colpopoiesis the uterus was preserved in 85% of patients. Analysis of the contents of the uterine cavity showed endometritis, which was evidently caused by the absence of the cervix uteri, a natural barrier preventing the penetration of intestinal flora of neovaginal mucosa into the uterine cavity. The studies permitted a new interpretation of the results of abdominal and sigmoidal colpopoiesis with preservation of functioning uterus in adolescent girls with vaginal aplasia.

RESUMO

Seventy-nine patients with sexual development retardation were examined, 24 of these suffered from ovarian genesis condition and 55 from central genesis condition. The findings evidence that detection of the uterus and gonads presenting as cords is one of the diagnostic criteria indicating gonadal dysgenesis. Echographic examinations carried out over the course of therapy yield a more accurate picture of ovarian function. No increase in uterine size on the echogram after discontinuation of hormonal therapy and the appearance of follicles in the ovaries after treatment point to normally functioning ovaries and helps specify the origin of sexual development retardation. In sexual development retardation of a central origin ultrasonic scanning helps assess the therapy efficacy and predict its outcome.

RESUMO

A study in 30 patients with an obstructed rudimentary vagina has demonstrated that sonography offers an early diagnosis and adequate management of patients with unilateral menstrual flow obstruction. It prevents an infection of the internal genitalia and results in complete reproductive rehabilitation of the patients. Renal sonography indirectly confirms the diagnosis since an aplastic kidney is located on the side of the accessory obstructed vagina in 100% of these patients.

RESUMO

Fifteen girls aged 12 to 18 years have been evaluated and operated for unilateral hematocolpos related to uterus didelphys with a partially aplastic vagina. It most commonly occurred at puberty and characteristically presented as the onset of algomenorrhea at a few months following menarche, with deterioration of it during every menstruation, and suppurative discharge refractory to any therapy. The most revealing diagnostic approach was the gynecologic examination supported by pelvic and renal ultrasound scans. Surgical approach to this malformation consists in dissection of the walls of the rudimentary vagina.

RESUMO

The purpose of the investigation is to elucidate the role of adrenal androgens in the development of the syndrome of polycystic ovaries in girls of pubertal age. Clinical manifestations of disease and hormonal indices, the levels of testosterone, 17-oxyprogesterone and cortisol with ACTH and dexamethasone tolerance tests were investigated. A significant rise of the level of testosterone and 17-oxyprogesterone was revealed in response to ACTH administration in 30% of the examinees indicating the adrenal genesis of hyperandrogenism. A characteristic feature of the development of disease in this group of patients is early development of hirsutism followed by disturbed menstrual function.

RESUMO

The study group included 49 patients aged 16 to 19 years with combined hemostatic and reproductive disorders (n = 26) and reproductive disease without hemostatic defects (n = 23). The study yielded a differential diagnostic protocol for juvenile uterine bleedings, pathogenetically relevant therapy and echographic monitoring of its efficacy.
